For you to register at this practice it is necessary for you to complete the two sets of forms below. Print off both sets of forms and these must have all sections completed on every page, although we do not expect you to know your CHI number or your NHS number so do not worry about these sections. Once you have completed all 6 pages please bring these, along with some form of ID, to the surgery so we can check these and see if it is possible for you to register at this surgery.
Practice Area
We accept patients living in the postcode areas of G41, G42-9, G43 and G44. Please remember to let us know of any change of address or telephone number. Patients who move out with the practice’s area will need to register with another GP and may be removed from our list if they do not re-register.
Patient Rights & Responsibilities
Patients can expect to be treated with courtesy in all dealings with the practice. In return, we expect patients to attend booked appointments or cancel if no longer required. In the event of a patient being violent or abusive to any member of staff, we may call the police and will remove the patient from our practice list.
Your medical records may take several weeks to arrive at the Practice.

Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
